We are looking for an experienced monitoring officer to work with the elderley for our client. The successful candidate will be based in the community at Bracknell, with visits to the Sutton office when required, you will deliver a high quality assured domiciliary care service that meets the needs of our customers and complies with the terms of our contractual obligations.

Summary of Duties and Responsibilities:

You will work with the Senior Co-ordinators to ensure all service needs are met both within the community and within the centre for all customers and purchasers of care.

Promote and maintain effective working relationships with clients, their representatives, local authorities and other purchasers and potential purchasers.

Complete the initial assessment of new clients including completion of risk assessments and care plans.

Undertake and ensure through working with Senior Care Workers that reviews of care plans and risk assessments are carried out to meet the changing needs of the client and to ensure compliance with the contract. Reports should be run every month to show the forthcoming monthly period clearly showing risk assessments, spot checks, PR&D's for Care Workers and other related work that the senior care worker must carry out.

To ensure recruitment needs are monitored and addressed including advertising, interviewing and liaising with HR and the training team for induction.

Ensure frequent contact with and arrange diary appointments for Senior Care Workers.

Regularly appraise the quality of the care delivered by Senior Care Workers through spot checks ensuring Anchor Care's quality procedures are met and wherever possible exceeded.

Under the direction of the Team Manager undertake investigations into complaints in accordance with Anchor Care's procedures e.g. conduct at Work.

Responsible for ensuring reviews are carried out from the care plans due to end report.

To monitor the 'Actual hours' reports and to present reasons for fluctuations in hours to the Team Manager on a daily/weekly/monthly basis.

To work with the Senior Co-ordinator to ensure there are no clashing calls by liaising with Senior/Care Workers and dealing with rota issues.

Ensure the promotion of effective communication with members of staff, clients and their families and friends, purchasers and other agencies in the local community to promote a positive image of Anchor Care.

Provide an assessment of the on-going training needs of Senior Care Workers and where necessary deliver or source the training to ensure they are equipped with the skills necessary to meet the changing demands of the service.

Undertake the recruitment, selection and induction of Senior Care Workers and any other posts as directed in accordance with Anchor Care's procedures.

To carry out regular supervision of staff through PR&D's every 13 weeks and 1-1's to ensure professional development is encouraged at all times. To ensure all PR&D information is passed to HR to log and to ensure all training needs from the PR&D are collected and shared with HR and the Training Manager for future planning.

Hold regular team meetings with Senior Care Workers and Care Workers as well as taking part in meetings with Senior Co-ordinators and Team Managers as directed.

Assist in the development of marketing processes to enable the centre and Anchor Care to develop its services in particular to private clients and for live-in services.

Assist with the development of business plans. Set, monitor and deliver on targets for Senior Care Workers. Deliver on business KPI's . The ability to utilise software and technology appropriately.
